Collection Site Policies and Expectations

To ensure a safe, professional, and compliant experience for all clients, the following policies apply to all appointments

at this collection site.

Appointments & Arrival

All services are provided by appointment only. Clients may not arrive more than five (5) minutes prior to their scheduled appointment time. A fifteen (15) minute grace period is allowed for the tech and client. Appointments may be cancelled after this time.

Cancellations & Refunds

Appointments cancelled due to late arrival (after the 15 minute grace period) are not subject to refund eligibility unless MDD caused the delay. Rescheduling is subject to availability and must be done at least 2 hours before your appointment time.

Payment Policy

No cash payments are accepted. All payments must be made using approved digital payment methods prior to appointment or at the time of service.

Visitors & Accompaniment

Only the individual being tested is permitted inside the collection site. Minors must be accompanied by a legal parent or legal guardian. Additional guests, children, or observers are not permitted.

Conduct & Safety

Abusive, aggressive, or disruptive behavior will not be tolerated. The collection staff reserves the right to refuse or terminate services if safety is compromised.

Privacy & Compliance

All collections are conducted in accordance with applicable privacy laws and chain-of-custody requirements. Client information is handled confidentially and securely.
